Request an Appointment
You’ll want to bring the following information with you to the consultation appointment:
- Your referral slip and X-rays if applicable
- A list of any medications you are taking
- Any known allergies or medical conditions
- Medical or dental insurance to be filed
If you’ve had X-rays taken in the last six months, please have your dentist forward them to us. If there isn’t enough time to get them to us prior to your appointment, please make arrangements to pick them up and bring them with you to the appointment.
A parent or guardian must accompany all patients under the age of 18. If you’re bringing a child in for the first time, our pediatric dentistry team will make sure they feel comfortable from the start.
If you have any questions about what to expect on your first visit, don’t hesitate to contact us today at (321) 447-4444. You can also review our new patients page and financial information page for more helpful details before you arrive.
